We welcome new patients. Download the forms to have on hand when you come in to visit.

Your first appointment with us will be an opportunity to express your concerns and “co-discover” your dental needs during a thorough examination, intra-oral photographs and diagnostic x-rays.

In order to complete this we allow one hour for this visit. We are then able to schedule the appropriate amount of time for cleanings and other services.

As a new patient, we ask that you complete the attached forms before your appointment. This information will allow us to be prepared for your initial visit.
